Medications
Only prescribed medication may be administered at school. If medication needs to be given, the following items are required:
A prescription form from the physician stating the child's name, medication, dosage and designated times or conditions when the medication should be administered.
A permission form signed by the parent/guardian giving the school the authority to administer the medication to the child.
The medication in its original, labeled container from the pharmacy. If given at home as well, the pharmacist will label a second container to be kept at school.
If a nebulizer medication is to be administered, a tubing with mouthpiece or mask should be sent in.
**All medications must be brought in to the health office by an adult. No medication should be sent into school with your child. Cough drops and over the counter medication are not allowed to be given.
*** At the time of drop-off, the permission form can be signed by the parent/guardian and medications will be counted.
